Using past perfect in both clauses is uncommon and usually unnecessary. Since past perfect describes an action that happened before another past action, you need a reference point in past simple. "She had eaten before he had arrived" is technically possible but sounds unnatural — "She had eaten before he arrived" (past perfect + past simple) is clearer and more standard. The exception is when both actions happened before a third implied reference point: "They had planned the trip and had booked the flights (before something else happened)." In practice, one past perfect verb is usually enough per sentence. In exams, the standard pattern is one past perfect clause plus one past simple clause.
